Requirements must be received 60 days prior to visit:

1. Copy of CV
2. Signed HIPPA Form
3. Current TB within the year
4. Current Flu immunization
5. Hospital Paperwork:
   • Clinical Experience Request Form
   • Orientation Acknowledgement (attached)
   • Confidentiality & IT Systems Use Agreement (attached)
   • Headshot/picture for ID Badge
