Documentation Required for Visitation

1. Copy of CV
2. Photo ID
3. Immunizations: submit either copies of laboratory titers or proof of vaccination
   Rubeola (Measles)
   Mumps
   Rubella
   Tetanus/Diphtheria/Pertussis
   Hepatitis B surface antigen and surface antibody
   Varicella
   Influenza (during current flu season)
   Current TB within a year
4. International Visitors will need to meet with the Immigration Department at NYU HJD prior to observation.
